Question

When performing a Ga-67 scan for sarcoidosis like the one shown, how long after injection are whole-body images typically acquired?

  1. AImmediately after injection (within 30 minutes), while the tracer is still in the blood pool and distributing to inflamed lymph nodes
  2. B24–72 hours after injection, to allow background blood pool clearance while Ga-67 remains in inflammatory and lymphoid tissues
  3. C4–6 hours after injection, when tracer has reached peak concentration in mediastinal lymph nodes before redistribution begins
  4. D7–10 days after injection, because Ga-67's half-life requires extended waiting before counting rates are adequate for whole-body imaging

B. 24–72 hours after injection, to allow background blood pool clearance while Ga-67 remains in inflammatory and lymphoid tissues

Ga-67 citrate has a physical half-life of approximately 78 hours and requires time to clear from the blood pool and non-target tissues before imaging. Whole-body images are typically acquired at 24–48 hours post-injection, with additional delayed images at 72 hours if needed to improve target-to-background ratio or clarify findings. Imaging too early results in high blood pool background that obscures lymph node uptake. Unlike Tc-99m agents (imaged within hours), Ga-67 studies require the patient to return on a separate day for imaging.
